Advanced Primary Care Management (APCM) is a Medicare program designed to provide comprehensive, coordinated care for your health needs. Hattiesburg Clinic offers this program to help ensure you have a dedicated doctor and care team to support your well-being.

What You Get with APCM

  • Patient Consent – You agree to be part of this program. You may stop these services at any time.
  • Initiating Visit for New Patients – We will schedule a visit with you if you are new or have not been seen by us in three years.
  • 24/7 Access to Care and Care Continuity – You can reach someone from your care team anytime, day or night, if you need help. Hattiesburg Clinic offers expanded hours at our Immediate Care facilities. You and your care team will work together to plan regular checkups.
  • Comprehensive Care Management – Your care team will help with your medical and mental health needs, including medication management and supporting you to take care of your health.
  • Patient-Centered Comprehensive Care Plan – We will create a care plan tailored to you that you and your care team can view anytime in Iris.
  • Management of Care Transitions – If you go to the emergency room or hospital, we will help with scheduling timely follow-up appointments with your care team.
  • Practitioner, Home and Community-based Care Coordination – We will coordinate services with other health care practitioners to help you achieve optimal health.
  • Enhanced Communication Opportunities – You can communicate with us using our patient portal, Iris.
  • Patient Population-Level Management – Hattiesburg Clinic has a care team working behind the scenes identifying opportunities to improve your health and the health of patients in our community.
  • Performance Management – Your care team works to provide you with high-quality care in a cost-effective manner.
