Hattiesburg Clinic Expands Support for Medicare Beneficiaries Through Advanced Primary Care Management

Hattiesburg Clinic is expanding its approach to patient-centered care with the Advanced Primary Care Management (APCM) program, offering Medicare beneficiaries additional support through a more personalized, proactive and coordinated care experience.

APCM focuses on strengthening communication, improving coordination and providing ongoing support for a wide range of health needs. Patients enrolled in APCM receive 24/7 access to their care team, ensuring their needs are met in between office visits.
