Did you know Hattiesburg Clinic has a service dedicated to patients with chronic medical conditions?

What is a Chronic Medical Condition?

The U.S. National Center for Health Statistics identifies a chronic medical condition as a condition or disease that persists for a long time, typically lasting three months or more. According to the Centers for Disease Control and Prevention, some of the most common, costly and preventable of these include heart disease, stroke, cancer, type 2 diabetes, obesity and arthritis.

If you are a patient with two or more ongoing medical problems, Hattiesburg Clinic wants to ensure that you have a clinic resource to help monitor your overall health. With Chronic Care Management (CCM), you are assigned a Case Manager Nurse who will work directly with your provider to keep your medical problems well controlled. Your nurse will assist in the many aspects of your health, including: preventative care services; self-management of the diseases; lab workups; radiology films; and follow-up appointments that are a part of treating your chronic medical conditions.

What You Should Know


We will bill your insurance directly for the services provided by your Care Manager Nurse as part of the CCM program. However, you might still be responsible for any deductible amount and/or a small copay of about $8 each month, even if you have secondary or supplemental insurance.

Note: This only applies to certain insurance plans or payers. Please contact your insurance provider to find out if this program is covered by your insurance.


Once you are assigned a Case Manager Nurse, he/she will contact you at least once a month to discuss your current status and health goals. You are not required to come into the office or have an “appointment” to speak with the Case Manager Nurse.


Prior to being assigned a Case Manager Nurse, you must be seen by your primary care provider. To participate in this program, a beneficiary consent form must be completed. Please speak with your primary care provider about enrolling.
